HERES WHAT YOU NEED TO APPLY FOR THE SCHOLARSHIP:

1. Proof of Tribal Enrollment

2. Proof of Enrollment in School (Tuition Bill or Class Schedule)
3. A Signed Letter from the student requesting Financial Assistance

for School and where the funds will go
4. Denial Letter from Higher Education
5. Signed Release Form (See O&G Dept. for form)

(For Verification, Research, & Processing of Application)
